Question:
I had a car accident and lost/fractured/root canaled 10 top teeth 28 yrs ago. First bridge lasted 17 years until front root fractured and broke. Second bridge lasted 9 years until front porcelein fractured and broke. I'm in process of 3rd bridge.

I'm in temp cement for permanent bridge (testing) and the bridge won't stay in for more than a few days. This also happened with the temp bridge. I feel that I can't permanently cement until I know why bridge keeps falling out. I'm also doing bottom bridges this time to fill in missing teeth. Still in temps in bottom.

Permanents have not been completed, still in process (more reason for not having cementing the top). Why does the top keep falling out? I've been given a nightguard because of possible grinding (very unlikely in my opinion).

I never had this problem with previous bridges and temp cement. Dr. says perhaps something is off but until we have permanent bottoms we may not know what. What should i do? ...Visitor from NJ

Answer:
It sounds like you've sure had a lot of dental treatment over the years. Fortunately from your description the past treatment had worked out very successfully.

It's hard to say why your new upper fixed bridge will not stay in with the trial cement. The provisional or temporary bridge also should not have given you this trouble. I wonder if this is the same dentist who achieved such a successful result the first two times? Or are there that many fewer teeth supporting this bridge? Were implants suggested to make up for some of the missing teeth?

I don't blame you for being wary of cementing the bridge with a hard or definitive cement (I try not to say permanent as you know nothing is forever!); I would like to know that the bridge stays in comfortably and successfully before committing to the hard cement.

If you don't reach satisfaction with your dentist I would seek a second opinion as it is obvious that you are undergoing major restorative treatment and you need to know it is being done the best possible way for the best and longest possible outcome.
