Question:
Bridge Pain - Sensitivity: Although I felt no pain initially following the installation of
a one-tooth bridge(upper right), after eating some almonds, I began to develop sensitivity in the bridge area as well as deep pains in the bottom right teeth.-- now persisiting for 2 months.
My dentist can't see a problem (he took x-rays), but he did reduce the occlusion anyway. While the endodonist confirmed sensitivity in the top teeth, he sees no other reason to perform root canals.
He prescribed mega doses of ibuprofen to reduce inflammation -- which has helped only slightly. Should I continue with the anti-inflammatories more than 2 weeks or insist that he do root canals on both top anchor teeth? ...Visitor from IL
Answer:
T-Scan makes a computerized occlusal analysis system. I have used it for years to diagnose bite problems. As the patient bites on the ultra-thin sensor, the T-Scan II software dynamically scans and displays the occlusion on the computer screen.
Teeth have two types of nerves. There are nerves within the tooth, and nerves around the tooth. The nerves with in the tooth are responsible for hot/cold/sweet sensitivity. The nerves around the tooth are responsible for proprioception (bite problems; the position of the body in space - that is how you know were your arms are with your eyes closed). Root canals only deal with the nerves within the tooth.
Before recommending root canal therapy, the bridge should be removed and an acrylic provisional placed to see if the symptoms subside. The acrylic provisionals are softer and will cushion the teeth allowing the inflammation to subside. The remaining tooth structure can also be checked for any fractures.
If it is a bite problem, the longer the teeth are traumatized the greater the chance they will require root canal therapy. Root canal therapy is not 100% successful, and it reduces the longevity of a tooth. However, if there is definitive reason to perform the procedure it is better than having the tooth removed.
There were no problems with the lower teeth prior to the placement of the bridge. Although it is possible that the almond caused a fracture, it is unlikely.
