What can you do to improve the quality of bone when you have good dimensions of bone but when you drill the bone it is loose and your fixture can't be tightend up to 15N? ...Dentist from Iran

Answer:
I am a general dentist, not a periodontist or surgeon. However, I know that no matter what the dimension of the bone, quality that can only allow you to tighten an implant to 15 Ncm will most likely result in a failure.

Methods to add better quality bone could be block autograft, block allograft, particulate grafting, or using titanium mesh with a mixture of autogenous bone and bovine mineral and a membrane, submerged for about 6 months.

I have had poor quality bone in one area when placing implants, and have gone to a nearby site to find better bone. A cone beam image of the bone would probably be very helpful in this situation to better see the bone quality.
